§ 54-404. Identification of applicants.

The application for a special business license to conduct an outdoor festival shall include the name, including aliases; age; residence; mailing address; and telephone numbers of each person making the application. If the application is filed by a partnership, the name, including aliases; age; residence and mailing addresses; and the telephone numbers of each partner shall be included. If the application is filed by a corporation, the application shall:

(1)

Be signed by the president, vice-president and secretary thereof and contain their residences, mailing addresses, and telephone numbers;

(2)

Contain the telephone numbers, the mailing addresses, and the street addresses of the principal place of business of the corporation; and

(3)

Include a certified copy of the articles of incorporation and the bylaws of the corporation.
